NOTICE OF PETITION FOR MODIFICATION AND DEVIATION FROM THE TERMS OF THE CLAUDE AND BARBARA CALDWELL MEMORIAL NURSING FUND

The Pulaski Memorial Hospital Auxiliary, Inc., as fund administrator of the Claude and Barbara Caldwell Memorial Nursing Fund, hereby gives notice to the heirs, beneficiaries or any other parties having interest in the Claude and Barbara Caldwell Memorial Nursing Fund, of the Petition for Modification and Deviation from the Terms of the Trust. In accordance with Indiana Rules of Civil Procedure 4.13, this constitutes service and notification.

The Pulaski Memorial Hospital Auxiliary, Inc., has filed the Petition in Pulaski Superior Court, under cause number 66D01-2605-MI-000018, for modification of the terms of the Trust. The Petition requests the Courts to modify the Administrator of the Trust. The Petition also requests the Courts to modify the qualification of scholarship recipients under the Doctrine of Deviation of Trusts. If any heirs, beneficiaries or other interest parties objecting to the proposed modification and deviation from the original terms of the Trust, you must contact the Court, in writing, before the Court Date. 

The Court has set a date of July 15, 2026 at 9:00 a.m. ET for a hearing on said Petition. All interested persons may appear and show cause of why the Petition should not be granted.
