REQUEST FOR PROPOSALS

Northwest Indiana Community Action Corporation of 5240 Fountain Drive, Crown Point, Indiana, which serves as the Area Agency on Aging for Jasper, Lake, Newton, Porter, Pulaski and Starke counties and as the Community Action Agency for Jasper, Lake, Newton and Porter Counties, will accept proposals to serve disabled, elderly and low-income families in Northwest Indiana. Successful proposals will result in service provider contracts beginning July 1, 2016.

Older Americans Act Title III annual funding of an estimated $2,932,000. Services include: Transportation, Homemaker, Legal Services, Congregate Meals, Home Delivered Meals, Health Screening, Health Promotion & Education, Medication Management, Caregiver Counseling, Caregiver Support Groups, and Respite Services and other pertinent services.

CHOICE annual funding of an estimated $2,090,000 and SSBG funding of an estimated $660,000. Services include: Adult Day Care, Attendant Care, Home Delivered Meals, Homemaker, Respite, Home Health Aide, Nursing, Home Health Supplies, Transportation, Personal Emergency Response, Handy Chore, Pest Control, and other pertinent services.

LIHEAP annual funding of an estimated $7,285,500. Services include Energy Assistance Intake, Client Education of Energy Conservation, Weatherization Shell and Weatherization Mechanical services and other pertinent services.

DOE annual funding of an estimated $165,000. Services include Weatherization Intake, Weatherization Shell and Weatherization Mechanical services and other pertinent services.

HUD annual funding of an estimated $125,000. Services include Housing Choice Voucher Program Intake and other pertinent services.

Proposal packets and technical assistance will be available according to the following schedule.

Weatherization Services: Packets available – by January 22, 2016; Technical Assistance Workshop – February 2, 2016; Proposal Packets Due: February 12, 2016

Nutrition Services: Packets available – by February 5, 2016; Technical Assistance Workshop – February 16, 2016; Proposal Packets Due: February 26, 2016

All other services: Packets available – by February 12, 2016; Technical Assistance Workshop – February 18, 2016; Proposal Packets Due: March 4, 2016
