BIG MONON DITCH WASHOUT REPAIR 135+00

WHITE COUNTY, INDIANA

Sealed bids will be received by the Pulaski County Surveyor, 112 East Main Street, room 130, Winamac, IN 46996 no later than 9:30 a.m. (local time) on August 26, 2016 and will be publicly opened and read aloud at the Joint Drainage Board meeting that morning at 10:00 a.m. (Eastern Daylight Time). The proposed project is generally described as:

Big Monon Washout Repair 135+00

Any bids received after 9:30 a.m. will be returned unopened. A pre-bid meeting for all divisions of work will be held at 9:00 a.m. (Local Time) on Friday July 29, 2016 at the County Courthouse, 112 East Main Street, Winamac, IN 46996. All prime contractors, subcontractors, small, minority or women owned enterprises and other interested parties are invited to attend.

DESCRIPTION OF WORK:

Shape bank and install 925 lf of rock toe and stone drain fill. Stabilize with erosion control blanket and apply seed mix indicated on construction plans. Estimated quantities include 7,030 tons 6”x12” Revetment riprap, 760 tons INDOT coarse aggregate #8 filter stone, 670 cy INDOT fine aggregate #23, 1,300 sy geotextile, 5,000 sy erosion control blanket, and 1± acres of seed.

BID DOCUMENTS: Copies of the Specifications and Contract Documents for all divisions are available for examination at the Pulaski County Surveyor’s Office, 112 East Main Street, Winamac, IN 46996 beginning at 8:30 am local time, July 18, 2016. Copies may be obtained at the Pulaski County Surveyor’s Office by remitting fifty dollars ($50) for one complete set. This remittance is not refundable. Payment shall be by money order or check and shall be made payable to Banning Engineering. Copies may also be obtained by sending check or money order to Banning Engineering, Attn: Kevin Steely, 853 Columbia Rd, Suite 101, Plainfield, IN 46168. Bidder shall pay shipping costs. Electronic copies will be available at no cost.

The work to be performed and the proposal to be submitted shall include sufficient and proper sums for all general construction, mechanical installation, labor, materials, licenses, insurance, bonds and so forth incidental to and required for the construction of the facilities.

The proposal form must be enclosed in a sealed envelope bearing the title of the Project, the name and address of Bidder. All proposals must be submitted on the proposal forms as identified in the Contract Documents and Specifications.

INDIANA REQUIREMENTS FOR BIDS OVER $100,000: Standard questionnaire Form 96 shall be completely filled out and signed, including attachment of Contractor’s Financial Statement as stipulated in Section III. An IRS W-9 form is required.

Bids shall be properly and completely executed on Bid Forms and in the order as identified and contained in the Contract Documents obtainable from the office named above. Each bid shall be accompanied by an acceptable Bid Security. Any bid not accompanied by the above-required items shall be deemed a non-responsive bid.

No Bidder may withdraw his bid within a period of 60 days following the date set for the receiving of bids. The Owner reserves the right to retain any and all bids for a period of time not more than 60 days and said bids shall remain in full force and effect during said time. The Owner further reserves the right to waive informalities and to award the Contract, with or without alternatives, to any Bidder all to the advantage of the Owner, or to reject all bids. 

A conditional or qualified Bid will not be accepted. Award will be made to the low, responsive, responsible bidder. All applicable laws, ordinances, and the rules and regulations of all authorities having jurisdiction over construction of the projects shall apply to the Projects throughout.

BID SECURITY: Each bid shall be accompanied by an acceptable certified check and made payable to the Monon Ditch Joint Drainage Board or an acceptable bid bond in the amount equal to five percent (5%) of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the state of Indiana and whose name appears on the most current Treasury Department Circular No. 570. 

BONDS: A Performance Bond and Payment Bond each in the amount of 100 percent (100%) of the Contract price will be required from the Contractor to whom the work is awarded.
