NOTICE OF DISSOLUTION

Apostolic Christian Retirement Home, Inc. and Apostolic Christian Village, Inc. (“Corporations”) voluntarily dissolved on September 1, 2017, do hereby publish notice of their dissolutions and request that persons with claims against the Corporations present them in accordance herewith: A claimant must submit its claim in writing to Brian D. Casserly, 250 Main Street, Suite 590, Lafayette, IN 47901, and specify the following: claimant’s name, address, amount of claim, and description of claimed services and dates thereof.

Claims against the Corporations will be barred unless a proceeding to enforce the claim is commenced within two (2) years after the publication of this notice.
